New Patient?

Dr. Winstead & staff enjoy what they do and hope you feel comfortable trusting us with your dental needs.
Call or Text our office at 503-399-7718 to schedule an appointment!

What to expect before & during your initial dental examination:

Prior to your initial examination:
  • After scheduling your initial examination, you will receive a welcome email invitation requesting to set up your patient portal. The link in this email to create your portal is only good for 24 hours, so if the link expires you can always text/call the office requesting a new link be sent. The login for the patient portal will be your email address and it will prompt you to create a password (following the correct password guidelines).
  • Once logged into your patient portal you will find all of your New Patient forms that need to be filled out prior to your initial appointment. If you are switching care from another dental office that we will need to request records from, we ask that you complete your forms as soon as possible so we can request those records in a timely manner prior to your first appointment with us.
  • If you have dental insurance you would like us to bill, you can fill out that information on your Patient Information form. We also request that you please text/email the office a picture/copy of your dental insurance card prior to your initial appointment so we can verify your insurance ahead of time. If this is not an option for you, you can bring your dental insurance card with you to your first visit, and can make copy at the office.
Paperwork

Your first visit
  • The initial dental examination will take approximately 1 hour.
  • An evaluation of your gums and teeth
  • Dental x-rays
  • Schedule a review of findings ("ROF") appointment

Your "ROF" appointment will be shorter in length and the focus is to provide an analysis of your oral condition and will prioritize a treatment sequence for you.  After your ROF appointment, we can then schedule any necessary treatment including a cleaning.
